HRA eligible expenses: what can be reimbursed?

An expense must meet the tax rules, the rules for your HRA type and your employer's plan terms before it can be reimbursed. Use this guide to check premiums, medical bills and documentation.

The short answer

An HRA may reimburse qualifying medical expenses allowed by its specific plan. A tax-eligible expense is not automatically covered by every HRA. Insurance requirements, expense restrictions, dates, available allowance and proof all matter. The same expense cannot be reimbursed twice. source source source

How do I check whether an expense is eligible?

  1. Identify the HRA. Read the full arrangement name in the benefits notice or plan document.
  2. Confirm the covered person and insurance. Check whether you or the family member qualify and meet the applicable coverage conditions.
  3. Check the expense. It must qualify under the tax rules and be included in the plan's reimbursable categories.
  4. Check the date and allowance. The expense must fall within the permitted coverage period, and sufficient reimbursement benefit must be available.
  5. Provide proof and coordinate payments. Submit the required records and account for insurance payments or other reimbursement.

For HRA tax treatment, IRS Publication 969 and the arrangement-specific rules matter. Publication 502 explains many medical-expense categories, but it is principally a deduction guide: it is not your HRA's expense list or a substitute for its rules. For example, the medical-expense itemized-deduction threshold is not a threshold you must meet before claiming HRA reimbursement. source source

What kinds of expenses may qualify?

An expense already reimbursed elsewhereThe same dollars cannot be claimed again from the HRA.

Can an HRA reimburse health insurance premiums?

ICHRA: it can reimburse qualifying individual coverage or Medicare premiums under its terms, with the required coverage substantiation. A spouse's group policy alone is not qualifying ICHRA integration coverage. The employer may also include other eligible medical expenses. source

QSEHRA: it can reimburse permitted premiums and other eligible expenses, with minimum essential coverage required for tax-free treatment. Premiums paid pre-tax through a spouse's employer need special attention: the same amount cannot simply receive a second tax-free benefit. Ask the administrator how the actual payroll and reimbursement treatment works. source

Excepted benefit HRA: it cannot reimburse individual major-medical or Medicare premiums, and generally cannot reimburse group-health premiums except for COBRA or other continuation coverage. A group-integrated HRA follows a different set of integration and plan terms. Read the HRA type comparison rather than applying ICHRA premium rules to every arrangement. source

Does a $400 monthly allowance mean I receive $400?

Not necessarily. In a simplified example, suppose an ICHRA makes $400 available for the month, covers only eligible premiums and you have a substantiated $350 premium with no other payer. It can reimburse $350, not pay the unused $50 as spending money. If the eligible premium is $500, the reimbursement is limited to the available $400 and you still owe $100.

This example assumes the expense and person qualify, ignores carryover and other payment sources, and illustrates reimbursement arithmetic only. It is not an average allowance, premium quote or affordability result. Whether unused allowance remains available later depends on the plan and arrangement rules. source source

What documents do I need for reimbursement?

Expect to document the person receiving care or coverage, the nature of the expense, the service or coverage period, the amount and any insurance or other reimbursement. A premium invoice, itemized receipt or explanation of benefits may be relevant, depending on the claim. Your administrator should specify its requirements and secure submission method.

ICHRA also requires substantiation of qualifying coverage under its rules; QSEHRA has its own coverage and expense substantiation requirements. A benefit card does not by itself prove every purchase is eligible. Keep proof and follow the plan's claim and appeal procedures. source source source

Can a broader expense list affect HSA contributions?

Yes. An HRA that can reimburse general medical expenses before the relevant deductible can disqualify new HSA contributions. A premium-only or appropriately limited arrangement may have a different result. Do not broaden the HRA's expense list without checking the effect on employees who want to contribute to an HSA. source source
